June 4, 2010 Dear Patients and Friends, It is after much deliberation and thought that I am writing to inform you that I will be formally closing the practice of Personal Success Wellness and Weight Loss. This will be effective immediately, starting the business day of June 7, 2010. It is very important that you make arrangements as soon as possible to select a new physician ASAP to make sure you receive uninterrupted medical care. Please keep in mind that many physicians require an initial “get to know you” visit to establish a doctor-patient relationship. You may choose to contact your medical plan to obtain a list of physicians in the area who are eligible to become your physician. You can also reach out to the Travis County Medical Society for a list of physician names and telephone numbers. The A4M and the IFM are also alternative medical organizations that might offer physician referrals. These organizations are readily researchable via the internet. Please let us know if you wish to obtain a copy of your medical records. The administrative fee is $40 to pay for the labor and materials involved with duplicating and mailing the records. The record request process requires that a formal designated form be signed and dated. This form may be downloaded at the company website after June 7, 2010. You may fax, scan and email, or snail mail the client-autographed request to the contact information indicated on this letterhead. Kindly complete all record requests by July 15, 2010. Refills for critical health-sustaining medications will be available so that your supplies will be replete for the next 30 days. If you contact your pharmacy, they can request a refill from us via fax. Thank you for having chosen me as your physician. It has been my privilege and honor to serve you.
